Advertisements to appear in the convention program booklet may be purchased using this form. Three sizes of ad are available, Full page, half page and quarter page. Please choose the type of ad you wish to purchase. Indicate in the text box who you want to be acknowledged as the advertiser.

Note: the pages of AKA Convention 2011 program will be in color. The booklet will be in 4.25x11" format. Allow for 0.5" margins.
